Support Coordination:
Helps participants understand and implement their plan, connect with services, and build capacity to manage their supports.
Specialist Support Coordination:
Higher-level coordination for people with more complex circumstances and significant barriers to implementing their plan.

Plan Implementation & Connection:
Assisting you in understanding your plan budget, identifying suitable service options, connecting with providers, and establishing service agreements.
Specialist Support Coordination:
Higher-level coordination for participants with complex support needs or significant barriers to implementing their NDIS plan. We work collaboratively with participants, families, providers, and other professionals to strengthen support arrangements, address barriers, and improve service stability.
Crisis & Service Stability Support:
Responding to changes, service breakdowns, and emerging risks by coordinating appropriate supports and working collaboratively to restore stable and sustainable support arrangements.
Building Capacity & Independence:
Empowering participants to build confidence in directing their own supports, exercising choice and control, and navigating NDIS processes independently over time.
Monitoring, Reporting & Plan Reviews:
Monitoring the effectiveness of supports, identifying emerging needs or gaps, coordinating relevant reports and evidence, and supporting preparation for NDIS plan reassessments and reviews.
